Mrs Chan has several medical conditions listed below. You are on duty and doing patient care rounds of a night shift and find Mrs Chan on the floor in her private room dressed in a night gown. The last thing Mrs Chan remembers is getting up to the toilet and to fill the vase with water several hours ago, she said that she fell on her way to the toilet. She states she has pain in her right hip area and the right leg is rotated internally. Mrs Chan has a large piece of broken glass from the vase in her left thigh. Mrs Chan’s nightgown is wet with urine and there is a pool of urine on the floor. The primary survey is completed following the DRABC acronym: D – danger – check for any dangers that makes the surrounding hazardous. As there are broken glasses, the area needs to be cleaned so the environment is safe. R – responsive – check if the patient is responsive or unresponsive by using the COWS method (can you hear me, open your eyes, what is your name, squeeze my hand). Mrs. Chan is responsive, and she is able to answer questions. A – airway – you need to check that the airway is open and clear. If the airway is blocked, you can open airway by tilting the head back and use two fingers from the other hand to lift the chin. B – breathing – check for any abnormal breathing by watching their chest movements, listening for sounds, and feeling the movement of air as they inhale and exhale. C – circulation – check for any signs of severe bleeding. In Mrs Chan’s case, she may be bleeding severely as she has an embedded piece of glass.
